ACA Health Insurance Questionnaire
On March 23, 2010, the Affordable Care Act (i.e., ObamaCare) was signed into law by President Obama. The law includes, among other provisions, an individual mandate whereas all individuals (with certain exemptions) must have health insurance coverage of some type. (Note: Beginning in 2019, the individual mandate remains in effect (i.e., you are still required to have health insurance under the ACA law); however, the individual shared responsibility payment (i.e., the tax penalty for not having health insurance) has been repealed.) As such, WBWilson Tax Service has created an ACA Health Insurance Questionnaire and requests that all clients complete the questionnaire in order to provide WBWilson Tax Service with the necessary information to complete your tax return accurately. The questionnaire should be completed as is applicable to you (and your spouse, if applicable) and all dependents that you will be claiming on your tax return and included with your other tax documents. If you are using the SmartVault secure client portal to provide your tax documents to WBWilson Tax Service, you should scan the completed form into a PDF or JPEG electronic document and upload it, along with your other tax documents, into your SmartVault portal.
